• Pre-Registration
• Detailed Biography/Resume (of your ministry events, responsibilities, where you ministered, etc.
• Photograph of you in your dance attire
• Head Shot
• DVD/VHS of you ministering in dance
• $25 Processing Fee (money order Attn: Indica Mitchell, NO CHECKS)
• No refunds on or after membership cards and paperwork have been processed

Ordination Ceremony/Packet:

• Anointed with oil and prayed over individually
• Presented with ordination packet
• photograph throughout ceremony and after service
